Stress-Induced Cardiomyopathy Masquerading as Acute ST-Segment Elevation Myocardial Infarction

Myocardial stress can lead to a myriad of cardiovascular complications, and stress-induced cardiomyopathy is the predominant manifestation. Exogenous or endogenous hormonal excess, sepsis, tachycardia, physical emotional trauma neurohormonal catecholaminergic surges. Stress-induced often presents with chest pain, ischemic-like ECG changes, troponin elevation, wall motion abnormalities in echocardiography. It diagnosis exclusion, coronary artery disease needs be ruled out by normal angiogram as per guidelines. predominantly postmenopausal women presentation similar acute syndrome (ACS) due plaque rupture. We report case 72-year-old female who presented emergency room severe anginal pain without any preceding stress. showed lateral leads ST-elevation serial serum troponins were elevated. Emergent cardiac catheterization insignificant disease. Left ventriculogram echocardiogram moderately reduced left ventricular systolic function akinetic-hypokinetic mid distal myocardial segments basal contraction suggestive cardiomyopathy.
